Role:

• Culturally Sensitive Care:  Acknowledge and respect the cultural backgrounds of patients and their families. Understand and incorporate cultural nuances into the provision of care.
• Evidence-Based Nursing Care:  Utilize the latest evidence-based practices in nursing to ensure the delivery of high-quality and effective care.
• Engagement with Patients and Families:  Foster positive health promotion by actively engaging with patients and their families. Encourage a sense of personal control, emphasizing healthy lifestyles and preventative health measures.
• Clinic and Home-Based Care:  Provide nursing care and services either within one of the clinics or in the patient's own home, adapting your approach based on the specific needs and preferences of the individual.
• Support for Clinic Workflows:  Assist in clinic workflows, ensuring that established care plans are followed and coordinating with a multidisciplinary health team.
• Multidisciplinary Health Team Approach:  Collaborate with a diverse health team to ensure a comprehensive and coordinated approach to patient care.
• Person-Centered Nursing Care:  Prioritize person-centered care, considering the unique needs, preferences, and values of each patient.
